HVCC can assist in determining the types of agency services that are appropriate for the client and assist with engaging those services. We can also assist with coordinating current in home care service providers and monitor services.

HVCC can assist with bill paying, checkbook balancing, and consulting with the client’s accountant or Power of Attorney representative on behalf of client needs.

HVCC can attend professional and health care provider appointments to ensure a continuum of care for your loved one. We will facilitate communication between providers, the client, and the support team. HVCC can also arrange for durable medical equipment, safety inspections or home modifications.

HVCC will navigate insurance application, provide tracking and follow-up of claims, and address concerns over all types of insurance including health, life, long-term care, Veteran’s and Medicare.

HVCC can act as concierge to arrange for community services for the client including transportation, support groups, cultural activities, housekeeping, purchase of supplies, nutrition, simple home repairs and more.

HVCC will keep support team and professionals informed of well-being and changing needs of the client. We will also organize mediation between all members of the support team.

HVCC will coordinate downsizing and a move to assisted living or other residential solutions.

HVCC will aid a family with making Hospice or end of life (funeral) arrangements, as well as consolidation of residential affairs.

The stress of managing your care or that of a loved one can take a toll on all aspects of your life. We want to help alleviate that stress by providing support to navigate care for you or your care team members.
